Google’s fifth smartwatch is externally identical to the fourth and quietly the smartest small watch you can buy. The GPS work is remarkable, the price went up, and the headline health features still are not here.

The watch nobody stops me to ask about

I have been wearing Pixel Watches since the first one, and in four years not a single person has asked me what is on my wrist. That question gets asked about an Apple Watch Ultra. It gets asked about a Fenix, usually by someone who owns one. It does not get asked about this, and after five generations that has stopped being an accident and started being the most interesting thing about the product.

The unit I have been living with is the 41 mm LTE, Satin Pyrite aluminum case with the Olive Active Band, part number GA11566-DE-N. It is the small one, the quiet one, the version that disappears under a shirt cuff. And it is a strange object to review, because almost everything Google improved this year is invisible. The case is the same case. The screen is the same screen. What changed sits in a cloud pipeline, in a chip revision nobody will feel, and in a set of health features that were announced in August and, as I write this, still have not shipped.

What Google actually changed

Externally, nothing. Same 41 and 45 mm cases, same 12.3 mm height, same domed Actua 360 panel, same crown and side button, same Active Band mounting. Google did not even pretend otherwise at the announcement on August 12.

Inside, the list is short and mostly sensible. The chip is a Snapdragon W5 Gen 2 Accelerated with a Cortex-M55 co-processor, which Google rates at 12 percent more CPU and around 20 percent faster overall. RAM goes from 2 GB to 3 GB. Storage doubles from 32 GB to 64 GB. The 41 mm battery creeps from 325 mAh to 332 mAh, which is a rounding error rather than an upgrade. It ships on Wear OS 7.0, built on Android 17.

Then there are the two things that actually matter: a rebuilt GPS pipeline that reprocesses your track in the cloud after the fact, and Health Guardian, an umbrella for blood pressure trends, insulin resistance trends, sleep breathing quality, and breathing emergency detection. One of those four is live. I will come back to that.

Design, and the Olive band

The 41 mm case is 31 g without a band, which is light enough that I forget it is there during the day and light enough to sleep in without resentment. That last part is not a small thing, because every health feature Google sells depends on you wearing this to bed, and a 45 mm watch on a small wrist gets taken off at midnight.

The domed glass is still the defining choice, and it still cuts both ways. It looks like jewelry rather than instrumentation, the swipe from the edge feels natural in a way flat panels never manage, and it is a magnet for exactly the kind of doorframe scrape that ruins a curved surface. Aluminum at 41 mm has no bezel to sacrifice itself. Anyone who bumps their wrist into things should budget for a scratched dome inside two years, and no case will fix that on a display this shape.

The Satin Pyrite finish is the most interesting one Google has made. It is a warm brassy gold that reads as bronze indoors and much yellower in direct sun, and the Olive band, second generation this year, is a muted grey-green that keeps the whole thing from looking like a dress watch. The fluoroelastomer has a soft-touch coating that is genuinely pleasant against skin and genuinely a lint collector in a pocket. Small and large bands are both in the box, which is the correct decision and still not universal at this price.

Setup and the first hour

Open the box and there are three things in it: the watch, both band sizes under the tray, and the Quick Charge Dock. No power adapter, so the charging figures Google quotes assume you supply something decent. Feed it a phone charger you already own and the numbers hold up; feed it a five-year-old 5 W brick and they do not.

Pairing runs through the Google Pixel Watch app, and if you are coming from an older Pixel Watch the restore now carries watch faces, tiles, and app layout across, which it did not always do well. The eSIM step is where the LTE model gets particular. In Portugal, the LTE variant is a Vodafone exclusive, so the activation flow assumes a Vodafone line and the watch has to sit on the same operator as your phone for full call and message handoff. That is a Google requirement, not a Vodafone one, but it means the LTE decision here is really an operator decision.

The first hour after that is mostly Google Health asking permission for things. Give it everything or the watch spends a week half-informed.

Wear OS 7, Gemini, and the September build

This is the most finished the software has ever felt. The September update, build CP3A.260905.002, landed on the first of the month and is meatier than most annual releases: offline Gemini for core actions, a much smarter double-pinch gesture, and the corrected GPS route maps in the Health app.

Offline Gemini matters more than it sounds. Setting a timer, starting or stopping a workout, and toggling Do Not Disturb now run on-device with no round trip, which means they answer instantly and they answer in a basement car park. Every voice assistant on a watch has always failed in exactly those two situations.

The gesture work is the part I did not expect to use and now use constantly. Double pinch opens At a Glance or the notification shade from the watch face, scrolls Gemini answers, cycles Wallet cards, and pages through workout metrics mid-run. One-handed operation of a watch sounds like a party trick until you are carrying groceries.

Gemini itself remains the clearest lead Google has over anyone. Ask it something in plain speech and it answers with your calendar, your mail, and your location already in hand, on a screen the size of a coin. Nothing else on a wrist is close. The caveat is that the good stuff in the Google Health app, the coaching, the written insight, the interpretation of your own data, sits behind Google Health Premium. Without it the app is a competent chart viewer. With it, it is the best health platform on any watch. That is a subscription conversation dressed up as a hardware review, and it belongs in the buying decision.

The GPS work, and what it does not fix

Google rebuilt outdoor tracking, and it did it somewhere unexpected: not on the watch, but in the cloud, after you finish.

Three corrections get applied. The first uses Google’s 3D model of the world, the same building and terrain heights behind Google Earth, to work out which satellites your watch could actually see from where you were standing, and to throw out the reflections bouncing off the glass tower across the street. The second pulls real-time atmospheric data from reference stations to correct for weather. The third re-runs sensor fusion, the accelerometer and gyroscope sanity check, against both of the first two.

The result in dense city streets is the best I have seen from any watch. Where a Fenix puts you on the wrong side of the road and an Apple Watch quietly snaps you to the nearest sidewalk, this puts you where you actually were, including when where you actually were makes no sense. Run a circle in the middle of a closed street and it draws the circle. Walk under a bridge, double back, and cross over, and it follows you. The Baixa grid in Lisbon, six storeys of stone on both sides of a four-metre street, is precisely the problem this was built to solve, and it solves it.

Now the honest part, because this is what everyone is arguing about. It is post-processing. Your live pace, your live distance, and your on-watch map during the run are untouched. The corrected track appears in the Google Health app up to ten minutes after you stop, and until then you see the raw one. If you were hoping for better guidance while running a trail, this does nothing for you. If your objection is that a prettier line afterward is vanity, that objection is fair on a track workout and wrong on anything you intend to look at again. Route data is what people actually keep.

Out in forest and open gravel, the gains shrink to nothing much. Under cliffs and tree cover it is fine, no better than the competition, occasionally slightly worse. The headline is real and it is narrow.

Health, and the features that are not here yet

Health Guardian was the reason to buy this watch over last year’s, and most of it does not exist yet.

Blood pressure trends, which estimate the persistent direction of your pressure from pulse and motion patterns with no cuff, need a month of data before they report and were promised for September. Insulin resistance trends, a monthly metabolic read, same story. Sleep breathing quality, same. All three are trends and monthly summaries rather than readings, all three are framed as wellness rather than diagnosis, and all three are also coming to the Pixel Watch 4, which weakens the case for upgrading considerably. As of today they have not landed, and the September changelog does not mention them.

The one Health Guardian feature that is live is the important one, and it is live here specifically. Breathing emergency detection watches for a persistent, critical drop in blood oxygen, confirms you are still and not simply asleep, and calls emergency services with your location if you do not respond. It is aimed at overdose, choking, and severe pneumonia. It is CE marked, it has no FDA clearance, and it is available in Portugal along with Spain, Italy, France, Germany, Ireland, Belgium, the Netherlands, Austria, Denmark, Norway, Sweden, Switzerland, and the UK. It is opt-in and off by default, so it does nothing at all until you go and turn it on, which you should do in the first hour. On an LTE watch it can place that call on its own, without the phone in the room. That single sentence is the best argument for spending the extra 100 euros.

The rest of the health picture carries over. Sleep timings are reliable, Google claims a 15 percent accuracy gain on stage detection, and the new smart wake alarm fires inside a 30-minute window before your set time when you are sleeping lightest. Bedtime mode dims the face to red on black so a wrist at 3 a.m. does not blind you. Cardio Load remains the best training load system on any consumer watch, because unlike Garmin it counts the whole day rather than only the hours you remembered to press start, and an afternoon carrying boxes lands on the chart where it belongs.

Where it still falls short for athletes

Optical heart rate is strong. On steady running and long rides it tracks close to a strap, and it holds through intervals with only occasional bobbles at the start of a hard effort. The sensor is unchanged from last year and it did not need changing.

The gaps are elsewhere and they are inexcusable at generation five. There is no manual lap. On a watch with 55 sport profiles, in 2026, you cannot press a button to mark a lap. You cannot pair a chest strap, so if you want a strap’s accuracy on a hard session you are into third-party apps and their attendant mess. There is no support for cycling power meters or any other external sensor. Data page customization runs to four fields on a single screen and that is the end of the conversation. There is no route navigation worth the name.

Google spent enormous engineering effort building the most accurate GPS track in the industry, then shipped it on a watch that cannot take a lap split. I have no explanation for that and neither does anyone else. The structured workout builder is good, especially when you describe the session out loud and the Health Coach assembles it, and the strength training interface with rep and weight tracking was announced for September and has not arrived either.

Battery life and charging

Google rates the 41 mm at up to 30 hours with the always-on display running and up to 48 hours in Battery Saver. Those are honest numbers rather than optimistic ones, and they describe a watch that is comfortably a full day and change, not a two-day watch.

In practice this is a nightly-charge device if you sleep-track, which is the whole point of it, and the 41 mm cell means you have less headroom than the 45 mm gives. What rescues it is the dock. Fifty percent in about a quarter of an hour changes the routine entirely: the watch goes on the charger while you shower, comes off nearly full, and battery anxiety stops being a thing you think about. That is a better answer than a bigger cell would have been, and it is why the 7 mAh gain over last year does not matter.

Turn on LTE and use it as a phone replacement on a long run and the picture changes fast. Cellular on a 332 mAh battery is expensive, and an hour untethered costs real percentage. Plan for that rather than being surprised by it.

Thermals, screen, and the everyday

The panel is the best part of the hardware. Three thousand nits of peak brightness means it stays legible in Alentejo summer sun with the sky white above you, and the 1-nit floor means the always-on face at night is a dim ember rather than a torch. The LTPO refresh drops to 1 Hz when nothing is moving, which is where the always-on display stops being a battery tax.

Heat is a non-issue in normal use. The case warms slightly on the charger and warms noticeably during a long LTE call, which is physics on an object this small, and it never gets to the point of discomfort against skin. The haptics are the best on any Wear OS device, sharp and short rather than buzzy, and the crown detent has a real mechanical click that the first two generations lacked.

Verdict

The Pixel Watch 5 is the best small smartwatch you can put on an Android phone, and it is a poor upgrade from a Pixel Watch 4. Both are true and they are not in tension. Google spent this year on a GPS system that genuinely leads the industry inside cities, on software that finally feels finished, and on health features that mostly have not shipped and will land on last year’s watch anyway. What you are buying today is a beautifully made, endlessly clever, properly repairable 41 mm watch that is quietly better at the things people actually do with a watch than anything else on Android, with a set of sport-tracking gaps that a five-generation product has no business still having.

Buy it if you are coming from a Pixel Watch 3 or older, if you want the small case, and if you want a watch that can be repaired instead of replaced. Pay the 100 euros for LTE only if you leave the house without your phone or if you want the emergency features to work when nobody is around; otherwise take the 419 euro Wi-Fi model and spend the difference on Google Health Premium, which is where half the value of this thing actually lives. Skip it entirely if you own a Pixel Watch 4, because everything worth having is coming to your wrist anyway. And look elsewhere if you take a lap split seriously, because Google still will not let you.
